Mailing List
Add your name and contact information to our mailing list. You will receive information concerning sign language classes, workshops and community events being hosted by the Center.

Interpreter Invoice
This form is to be completed by community based interpreters who provide VRI and Dynamic interpreting services on behalf of the Betty and Leonard Phillips Deaf Action Center. After completing the form click on the "Email Form" button.

Interpreter Testing Information Form
Each candidate for RID or EIPA testing should complete the Interpreter Testing Information Form prior to testing. The information taken from this form is used to complete paperwork necessary for testing forms forwarded to RID or Boy Town.

Sign Language Studies Registration Form
This form is used by studnets to register for Sign Language Classes and for workshops being offered by the Center.
